When you make your appointment, you will receive a pre-appointment information request email. This is about fact finding and includes you completing a new patient form, 4-day detailed food diary and an online nutritional assessment questionnaire prior to your appointment.
At the appointment I take a case history and discuss the information you have provided to ascertain your individual requirements. This is also when I will outline the treatment plan, protocol and any formulas, dietary adjustments or recommendations regarding functional testing you may benefit from.
The pricing plan for this is as follows:
New Patient appointment 60 minutes $175 (child / student / pensioner $130)
Testing price: depends on tests required / requested
New patient process for children is usually managed with a focus on the food diary and nutritional requirements and does not require completion of the online nutritional assessment questionnaire.
Payment policy: Payment is to be made at time of consultation and options available are either cash or bank deposit.
